Prospect left by everyone but still in the prospect tracker without rewards

We finished the exoctis (Payday: Extraction) Prospect, everybody left the prospect with their dropship but nobody received a reward and it still shows the prospect as open on the tracker.

I have similar problem but in my offline character, I just finished Incursion: Scan, offline private game. Game still shows in prospect in character select screen.

My group has now had this happen three times. VERY frustrating.

Same thing just happened again. This is the third time playing with friends and spending extra time to mine the exotic ore at the end of the mission and end up with zero exotic ore after returning to space. Every single time we have the prospect tracker with timer counting down. Very frustrating....
