open world cave entrance re-sealed

I play open world, 3 times now caves that i have previously opened have been re-sealed with stone that i needed to open again. Even caves that where open at start of prospect have sealed. Thankfully items inside cave have still been left inside.

This is still a problem on open world prospects where the cave has not been visited for a while.

third time reopening this one since this open world prospect was created. I can give you details if it would help you fix it.
