corpse loot disappearing or unreachable

There are times when you die, your items dont appear or they appear within objects. Twice I have noticed this happening when playing solo. Died in desert on a rock and when returning quickly after there was nothing left. Either its inside the rock or there was nothing left begin to begin with.
